If you are new patient, Dr. Madadala’s staff will take your information and setup your medical records in the Electronics Medical Records system. Once we have your information updated in the system you will receive a notification from the patient portal. This notification will go out to your email or your cell phone (text).
Once you receive this notification, please login into the Patient Portal and fill out the information. By filling out the patient information/forms in the portal prior to your appointment, you will save significant time during your visit at Dr. Yamini Maddala’s office. If you have any questions when filling out the information on the patient portal, please do the best you can, and our staff will assist you with any questions on the day of your visit.
Note: The patient portal credentials expire after a certain period, hence timely registration and login is required.
You will be still required to sign some forms/disclosures on the day of your visit in the clinic.
